Functional capacity assessments for your NDIS plan.
A clear picture of how things affect daily life, day to day.
A functional capacity assessment looks at how a disability affects everyday functioning, across areas like daily living, communication, social interaction, mobility and self-management, and sets it out in a structured report the NDIS can use for access requests and plan reviews. Wiser Minds provides functional capacity assessments for self-managed and plan-managed participants, by secure video across Australia.

Wiser Minds is not a registered NDIS provider. Functional capacity assessments are available to participants who are self-managed or plan-managed only. Agency-managed participants must access assessment through a registered NDIS provider. If you are unsure of your management type, check your NDIS plan or ask your support coordinator.

It is a structured assessment of how a disability affects everyday functioning, across areas like self-care, communication, social interaction, mobility and self-management. The result is a report that sets out your functional capacity in a way the NDIS can use.
A diagnostic assessment works out whether a condition is present, such as ADHD or autism. A functional capacity assessment describes how a condition affects day-to-day life. They answer different questions, and sometimes a plan needs one, the other, or both. We help you work out which on the free call.
Yes. The interview and functional measures are completed by secure video, with collateral information gathered from people who know you well where that adds to the picture.
Wiser Minds works with self-managed and plan-managed participants. If you are self-managed you arrange and claim the assessment yourself; if you are plan-managed your plan manager settles the invoice. We confirm how your funding applies on the free call before anything is locked in.
